This one-stage procedure transcends traditional methods, combining cancer removal and reconstruction in one surgery. Direct-to-implant breast reconstruction achieves immediate, aesthetically pleasing results, speeding recovery and a return to normalcy after mastectomy.
Traditional mastectomy and breast reconstruction involve several stages. The first is the removal of breast tissue during mastectomy. Tissue expanders are inserted and incrementally filled with saline over several months to stretch the skin and muscles and make room for future implants.
After the chest has healed and reached the necessary expansion, a second procedure is performed to exchange the expanders with long-lasting implants. Additional surgeries may follow to fine-tune the implants’ shape, size, and symmetry.
By comparison, direct-to-implant breast reconstruction involves only a single surgery. Implants are immediately placed after the breast tissue is removed through a small incision. When mastectomy and reconstruction are performed in one stage, the breast’s skin envelope can be spared, yielding aesthetically superior results.

Direct-to-implant breast reconstruction achieves excellent symmetry for patients undergoing bilateral mastectomies, which also makes the procedure an appealing option for patients opting for preventive mastectomy.
The procedure comprises 2 phases, each performed by a different surgeon. The first phase is the mastectomy, during which the breast cancer surgeon removes the breast tissue through a small incision made at the base of the breast to minimize scarring.
In the second phase, Dr. Cassileth or Dr. Killeen inserts the implant. A specialized tissue matrix is used to construct an “Internal bra” that supports the implant, allowing the surgeon to achieve a more natural breast position. The result is a beautiful, natural-looking breast immediately after surgery.
Like any surgical procedure, direct-to-implant breast reconstruction carries potential risks, which may include capsular contracture, infection, complications with wound healing, and issues related to anesthesia. Your surgical team will provide a comprehensive risk assessment and make sure you clearly understand the potential complications before surgery.
After your procedure, you’ll recuperate for 1 to 2 nights in a comfortable, luxe aftercare facility. Your chest will be bandaged and wrapped in compression garments and a suction dressing to reduce swelling and promote healing. You’ll have 2 drains, which will be removed 1 to 2 weeks after surgery.
Most patients return to work and their usual activities within 2 weeks, although we recommend avoiding heavy lifting and strenuous activity for 3 to 4 weeks.
